Here are twelve free REx-PN practice questions in the formats the exam uses, with the answer and a reason for every option. They use SI units and Canadian terms, and they need no account.

Medibara wrote these questions on 6 October 2026. They are original. None is taken or recalled from the exam. Each rationale names the reference it rests on. A nurse educator has not reviewed them yet, so if a rationale disagrees with your course material, follow your course and tell us.

How REx-PN asks questions
The test plan lists five formats: multiple choice, multiple response, fill-in-the-blank calculation, exhibit and graphics. Most questions are multiple choice. This set has nine multiple-choice questions, two multiple-response questions and one calculation.
- REx-PN has no case studies, bow-tie or matrix questions. Those belong to the Next Generation NCLEX.
- Multiple-response questions have earned partial credit since January 2026. Every other question is scored correct or incorrect.
- The exam has 90 to 150 questions and a four-hour limit. Thirty of the questions are unscored pretest questions.
- Lab values appear in SI units, such as mmol/L. Temperatures are in Celsius.

This set leans on Management of Care and pharmacology, the two largest areas. It has no questions yet on health promotion, psychosocial care or basic care and comfort.
